Make a Referral

If you would like to refer someone to Anna Westlake, Minds in Mind Occupational Therapy Consulting, please complete the referral form below with as much information as you can provide.

The information you share helps us gain an understanding of the person’s circumstances, current needs and goals, and how Occupational Therapy may be able to support them.

We welcome referrals from individuals, families, support coordinators and support teams, health professionals, organisations and other service providers.

If you have any questions about making a referral or would like to discuss a potential referral with us, please get in touch.

Please note, the referral will not be progressed until the following documents have been provided (where applicable) :

- Family Court Orders

- Guardianship and/or custody orders

- Case Plan (where there is child protection involvement)

- DCP Aboriginal Cultural Identity Support Tool (ACIST) if child is Aboriginal or Torres Straight Islander

- Previous psychiatric, medical or allied health reports
